Fluoroscopic X-Ray Guided Joint / Soft-Tissue Procedures

Fluoroscopic X-ray guidance used to target selected joints and soft tissues precisely.

Overview

Injections into joints, bursae, tendon sheaths, and other soft tissues are considerably more reliable when needle placement is confirmed in real time. Fluoroscopic X-ray guidance shows the needle tip against the bony and joint landmarks that define the target, so medication is delivered where it was intended and nearby structures are avoided.

The benefit is precision in both directions: medication reaches the structure that was actually identified as the pain source, and structures it should not reach are left alone. For a bursa or tendon sheath, the difference between an accurately placed injection and an approximate one can be the difference between benefit and a missed target.

Conditions treated

Considered for selected joint, tendon, bursal, ligament, and soft-tissue problems identified on examination: subacromial bursitis and rotator cuff tendinopathy, trochanteric bursitis, small-joint and periarticular problems, and other focal soft-tissue conditions where precise placement matters.

Who may be a candidate

Appropriate when examination has identified a specific structure as a likely pain source, when conservative measures including rehabilitation and load management have not provided adequate relief, and when the precision of image guidance would meaningfully improve either the diagnostic value or the treatment effect of the injection.

What to expect

You will be positioned comfortably, the target area is imaged with fluoroscopic X-ray to confirm the anatomy, and the skin is cleansed and numbed. The needle is advanced toward the target with continuous X-ray visualization of its tip relative to that target, and medication is delivered once placement is confirmed. The procedure typically takes 15 to 30 minutes, followed by brief monitoring.

Recovery

Most people go home the same day and resume usual activities quickly, with specific guidance depending on the structure treated; some tendons, for instance, warrant a brief period of modified loading. Soreness at the site settles within a day or two. As with other targeted injections, the symptomatic response over the following days is tracked and guides next steps.

Potential benefits

Precise placement supports diagnostic clarity, confirming the identified structure as the pain source, and can improve the treatment effect by delivering medication exactly where it is intended while avoiding structures it should not reach. Relief, when it occurs, typically supports continued progress in rehabilitation.

Risks and considerations

Bleeding, infection, and medication reaction are possible but uncommon. Temporary soreness and bruising are typical. For injections near tendons, the specific considerations of repeated steroid exposure to tendon tissue are discussed and factored into how often and whether to repeat. Lack of benefit is possible and redirects the evaluation.
